What is food allergy? A food allergy is an adverse health effect arising from a specific immune response that occurs after exposure to a given food. We do not need this form for medicine allergies, eczema or intolerances. This form MUST be signed by the parent & doctor. An email from the physician documenting the allergy is acceptable.
